Parent Letter #7 - Lunch Forms

Dear Parent/Guardian:

 

PS108 has been notified that new lunch forms must be completed to determine if your child will continue to receive free lunch.  On the reverse side of this letter is the letter we received directly from the Department of Education.  As noted in this letter the applications must be submitted by September 24, 2010.  If you do not submit an application you will have to pay full price for your child’s lunch beginning October 1, 2010!  All children will continue to receive free breakfast.  The new application is for lunch only.

 

The application is attached.  Each family will receive only one application.  This means all children attending PS108 must be listed on that one application.  If you need assistance filling out this form please come to the school between 9:00 and 10:00 am any morning beginning Wednesday, September 15 through Thursday, September 23.  We will have staff available to assist you.  There are directions noted on the inside of the application.

Please note that every family must submit an application before it can be determined who will be eligible to receive free lunch.  Remember, breakfast will continue to be free to all students.  If you are not eligible for free lunch, lunch will cost $1.50 per day or you may qualify for reduced lunch which will cost .25 cents per day.
