Parent letter #6 - Lunch Forms

Dear Parent/Guardian:


We have been notified that all parents must complete new lunch forms for the 2012 – 2013 school year to determine whether your children are eligible for free or reduced lunch.  Applications must be submitted by Monday, October 1, 2012. 


  • If you paid full price last year for your child’s lunch, you will continue to pay that amount until the new application is processed.
  • If you paid reduced price last year for child’s lunch, you will continue to pay that amount until the new application is processed.
  • If your child’s lunch was free last year, it will continue to be free until the new application is processed.


You must fill out a lunch application even if your child is going to bring lunch from home every day.  If you do not submit an application you will have to pay full price of $7.50 per week for your child’s lunch beginning September 6, 2012!  All children will continue to receive free breakfast.  The new application is for lunch only.


The application is attached.  Each family will receive only one application.  This means all children attending PS108 must be listed on that one application.  If you need assistance filling out this form please come to the school between 9:00 and 10:00 am any morning beginning Wednesday, September 12 through Wednesday, September 19.  We will have staff available to assist you.  There are directions noted on the inside of the application.

Please note that every family MUST submit an application before it can be determined who will be eligible to receive free lunch.  Remember, breakfast will continue to be free to all students.  If you are not eligible for free lunch, lunch will cost $1.50 per day ($7.50 per week) or you may qualify for reduced lunch which will cost .25 cents per day ($1.50 per week).
